Support Coordination is a Capacity Building support designed to help you understand, implement, and make the most of your NDIS plan. Your Support Coordinator develops a clear understanding of your goals and support needs and works with you to organise the right combination of supports to help you achieve your outcomes.
There are three levels of Support Coordination that may be included in your plan, depending on your goals, needs, and circumstances:
Support Connection : Builds your ability to connect with informal, community, and funded supports so you can maximise your plan and work towards your goals.
Support Coordination : Helps you develop the skills to understand, implement, and use your plan effectively. Your Support Coordinator works with you to build your capacity to manage services, strengthen relationships, live more independently, and participate in the community.
Specialist Support Coordination : Provides higher-level support for people with complex needs. A Specialist Support Coordinator helps you navigate challenging support environments and ensures your services are coordinated and delivered consistently.
